FIRST VISIT
We understand that you have choices regarding dental care - thank you for choosing Concord Smile!
During your first visit, we will gather all your necessary information. This includes a thorough review of your medical & dental history. You can download the Medical History Form and bring it with you, or come 15 min prior to your appointment to fill them out. If you have recent x-rays you can also bring them in to assist us in understanding your dental history better.

Out of town patients
Dr. Huynh is internationally known for her expertise in dentures and implants and has treated patients from all over the world at her office in Concord, CA.
When choosing a dentist for dental implants, dentures, or veneers, it’s important that you choose a dentist who can handle the job. It’s best to get dental treatment done right the very first time.
If you are interested in getting dentures, implants, or a smile makeover, and plan to come to our office, please notify us in advance as soon as possible, so we can accommodate you with our schedule due to the complicated nature of making dentures with so many appointments and lots of lab work in between.
